The band registration packet consists of a eleven forms, including a
registration checklist, most of which are required and some of which are
optional. Forms can be (1) printed and completed by hand, or (2)
completed electronically using the *fillable form feature and printed.
Please bring your completed forms to our first meeting for all band parents and students (marchers and non-marchers, including color guard) on Sunday, August 10 at 6:00pm in the auditorium. Band registration and other fees will also be collected at this meeting (checks payable to “WHSBBO”), if you have not already paid via PayPal through the web site. If you cannot attend the meeting, please turn in your forms and payment to the black lock box in the band room (located on the wall just to the left of the door as you enter the band room).
- Registration Checklist (Form A) – All band students (marchers and non-marchers, including color guard) complete this checklist to ensure your packet is complete and to calculate your total required payment for band fees and other expenses. Payment can be made by check payable to “WHSBBO” (acronym for Westfield High School Band Booster Organization) or via PayPal on the band website www.westfieldband.org
- Extracurricular Participation Policy (Form B) - All band students (marchers and non-marchers, including color guard) complete this form to participate in extracurricular band activities.
- Field Trip Permission Form (Form C) – All marchers (instruments and color guard) complete this permission form to participate in fall marching band activities involving travel (e.g., competitions).
- Emergency Care Form (Form D) – All band students (marchers and non-marchers, including color guard) complete this form, which will accompany students on band trips for use during an emergency.
- Medication Authorization Form (Form E) - A completed FCPS Medical Authorization Form is needed in order to administer any prescriptions, including OVER-THE-COUNTER medications, on band trips. This form is optional but highly recommended in the event your student gets a headache or other minor ailment during a band trip.
- Supplemental Medication Release Form (Form F) - While Form E is required to administer medications, we have found it difficult to use because of small print, lack of space, and lack of consistent format. To solve these problems, we developed this supplemental medication release form that presents this information in an easy-to-use format. This form is optional but greatly appreciated by our parent chaperones responsible for medications.
- Uniform Rental/Maintenance Agreement (Form G) – All band students that receive either a marching band uniform and/or a concert band uniform (essentially all band students except for color guard students who are not also in concert band) complete and sign this form describing your responsibility for uniforms and fees for lost of damaged items.
- Band Booster Membership/Sponsorship (Form H) – Complete this optional form to make a donation to the WHSBBO. All donations directly benefit our mission to help provide a quality educational program and to enable student participation in music competitions through scholarships and grants. WHSBBO is a 501(c)(3) non-profit organization. Your donations may be tax deductible as provided by law. Note that all parents/guardians are automatically members of the WHSBBO at no additional cost to you.
- Volunteer Interest Form (Form I) – We need your help to promote and support the WHS band program! This optional form will help you match your skills, interests, and level of desired time commitment to our band activities. There is no better way to stay involved with your kids, get to know their friends, make new friends yourself, have fun, and feel great about contributing to a truly wonderful and worthwhile program.
- Opt-Out Form (Form J) – Optional form to be completed only if your band student should NOT be featured in FCPS videos or photos.
- WHS Band Spirit Wear (Form K) – Optional form to purchase WHS band clothing (e.g., T-shirts, cinch bags, flip flops, jackets, and sweatshirts) for students and parents. Band chaperones and other parent volunteers are highly encouraged to purchase a band polo shirt for our parent “uniform” (black polo shirt and khaki slacks). Spirit Wear cannot be purchased through PayPal. Several orders will be placed throughout the school year. The first order will be due in the Band Room (Black box) by August 15.
