Registration Documents for 2024 - 2025

 ALL students in Band classes and activities, including Marching Band, Color Guard, and all Concert Bands are expected to complete the Westfield Band Registration Forms at the links below.

Marchers and Color Guard MUST have the Westfield Band Registration Forms (the google forms) filled in BY JULY 1st, 2024!!!  After registrating  pay the preseason Marching Band fee through My SchoolBucks (Westfield school store on My SchoolBucks, be sure to select the Marching Band Fee SY 24-25 ($100.00).   The FCPS forms can be turned in at band camp or bib fitting but the Registration Form needs to be done by July 1st!!

 Non-marchers may drop off the forms at band camp or bring into the first week of class. School fees should be paid on MySchoolBucks. Booster donations may be paid via check or cash.

Complete your registration by filling out the Google form and the four downloadable forms. Students should bring completed copies of the downloaded forms to the uniform fittings on July 31.. Most of the forms are Adobe fillable forms that can be completed and printed or saved. We recommend printing or saving a copy of the completed forms for your own records.

​School fees (marching band, marching band pre-season, instrument rental fees) should be paid online at www.myschoolbucks.com by “purchasing” them through the School Store. Booster donations and purchases should be paid via check or cash. **The band course materials fee will be assessed as an invoice to every student registered in a band class through your Student’s ID in MySchoolBucks in September and will be due by Sept 30.
Complete the Westfield Band Registration Forms on the Google Forms site and then download and complete the FCPS registration Packet and FCPS form files to turn in (the names are the links to the files):

1. Westfield Band Registration Forms can be filled out through the Google Forms located here.
2. ​FCPS Forms(located here) – a zip file with 4 Documents:
   1. 2024-2025 Field Trip form – this covers marching and concert related events. It needs one (1) student signature and one (1) parent signature.
   2. Emergency Care Information form – two (2) pages long, one (1) parent signature at the end required.
   3. Medical Authorization form – used in conjunction with the Supplemental Medication Authorization. Both forms must be signed by a parent in parts 1 and 2.
